Privacy Notice
Effective September 14, 2026. Abide Supportive Living, Inc. collects only the information needed to answer an inquiry, review an application, or deliver an authorized service.
General inquiries
When you call or email a general question, we keep your name, contact details, county, and the question so we can respond. Please do not send a participant’s name, date of birth, diagnosis, authorization number, or medical records by ordinary email; we will give you a protected way to share those only when a referral is actually being processed.
Job applicants
Application information is stored in Abide’s secured Google Workspace and used only for hiring decisions and the Wisconsin caregiver background check process. The first application never asks for Social Security numbers, I-9 records, tax forms, or banking information. Applicant records are kept as required by employment law and then securely destroyed.
Participants and families
Participant information is protected health information under HIPAA and Wisconsin law. Abide uses and discloses it only for treatment, payment, and operations—for example, to the county waiver agency that authorizes and pays for services—or as you direct in writing. You have the right to see and get a copy of your records, request a correction, request restrictions on certain uses, and receive a list of certain disclosures. A full Notice of Privacy Practices is provided at intake and available on request.
